For both 3-A Symbol and Replacement Part and System Component Qualification Certificates:

Renewal: Annual License Fee $1,025/calendar year

New - to estimate: Application Fee (one time) $750 and License Fee Multiply $85.50 x the number of full months from the date of application through December of the present year

Amendment: No Fee

For Process Certificates:

Renewal: Annual License Fee $1,025/calendar year

New - to estimate: Application Fee (one time) $750 and License Fee Multiply $85.50 x the number of full months from the date of application through December of the present year
